  • Surgery of the Brainstem

The ultimate guide to navigating and treating brainstem pathologies from master neurosurgeon Robert SpetzlerThe brainstem is one of the last bastions of surgical prohibition because of its densely packed ascending and descending tracts and nuclei carrying information to and from the brain. Although 10% of all pediatric tumors and 5% of all vascular anomalies occur in the brainstem, neurosurgeons have traditionally resisted dissecting lesions in this area. Recent advances in imaging, microscopy, anesthesia, and operative techniques have expanded the treatment paradigm for this most eloquent region of the brain.Surgery of the Brainstem, by internationally renowned neurosurgeons Robert F. Spetzler, M. Yashar S. Kalani, and Michael T. Lawton, along with an impressive cadre of global experts, is a comprehensive guide to managing disorders of the brainstem, thalamic region, and basal ganglia. Organized in seven sections with 33 chapters, the text opens with four sections covering a variety of topics. Section I presents the history of brainstem surgery; Section II examines anatomy, development, and pathology; Section III reviews patient examination, imaging, and monitoring; and Section IV provides a succinct overview of surgical approaches. Sections V-VII cover a wide range of adult and pediatric tumors, ischemia, stroke, aneurysms, arteriovenous malformations, and cerebral cavernous malformations. More than 300 high-quality clinical images and medical illustrations enhance the text.Key HighlightsA full spectrum of treatment modalities and outcomes, including open surgery, endoscopic approaches, stereotactic radiosurgery, radiotherapy, endovascular techniques, and revascularizationAn anatomy chapter featuring stunning Rhoton-style anatomical dissections delineates critical landmarks in the brainstem, thalamus, pineal region, and cranial nervesDetailed discussion of patient positioning and exposure of various brainstem domainsPearls on overcoming psychological, pathological, and anatomical barriers and managing complicationsUnderstanding the basic anatomy, pathology, and clinical complexities of the brainstem and thalamic regions is essential for safe navigation and treatment. This remarkable book will provide neurosurgeons with additional insights on performing resections and achieving the best possible outcomes for patients with pathologic conditions in this delicate region.

  • Appendicitis: Prevalence, Risk Factors and Clinical Management

Acute appendicitis is one of the most common surgical abdominal emergencies worldwide, with a lifetime risk of 8.6% for males and 6.7% for females. This compilation discusses how while non-operative treatment of appendicitis has been proposed as an alternative to surgery, appendicectomy remains the mainstay of treatment in most cases and techniques have shifted from open to laparoscopic methods. The current knowledge in pathogenesis, diagnosis, and clinical management of acute appendicitis is also discussed. Next, the authors present an updated review chapter of appendicitis regarding its prevalence and risk factors, particularly how the diagnosis of appendicitis is based on a history of diffuse abdominal pain, which usually starts in the central region of the abdomen and migrates to the lower right quadrant of the abdomen. The residual remnant or "e;stump"e; of the appendix after an initial appendectomy may become inflamed causing what is commonly referred to as stump appendicitis. Treatment with antibiotics may be attempted as an alternative to surgery; however, the vast majority of cases require definitive surgical excision of the remnant appendix. The authors propose that keeping this disease in the differential is important in reducing complications associated with delayed diagnosis.

  • History of Cardiac Surgery af Ugo Filippo Tesler
    - An Adventurous Voyage from Antiquity to the Artificial Heart
    af Ugo Filippo Tesler

The beginning of cardiac surgery is generally considered to be the successful repair of a wound of the heart that took place on the 7th of September 1896 in Frankfurt am Main. This operation put an end to the widespread belief that nature had set the heart beyond the limits of surgery. The successive development of heart surgery moved together with other advances that were rapidly taking place in various fields of medicine and surgery and which, already in the first half of the 20th century, had allowed surgical pioneers to successfully correct a number of congenital and acquired heart diseases by adopting closed-heart techniques. Undoubtedly, the most notable progress in the history of cardiac surgery took place between the second half of the 1950s and the end of the 1960s with the introduction of extra-corporeal circulation that allowed surgeons to perform interventions under direct vision within the bloodless heart chambers. This fundamental technological innovation fostered the development of surgical procedures that are still adopted to this day. Among these that must be mentioned are the correction of complex congenital heart diseases, the designing and creation of implantable prosthetic heart valves, the introduction of coronary artery surgery, the repair of severe diseases of the aorta, the commencement of heart transplantation, and the first implantation of an artificial heart. This book narrates these fascinating and sometimes dramatic events, as well as detailing some of the greatest pioneers of cardiac surgery.

  • Practical Approach to Asian Rhinoplasty af Yasushi Sugawara
    - How to Design A Fine Nasal Shape
    af Yasushi Sugawara

Difficulty in rhinoplasty takes two forms: the aesthetic and the technical. Rhinoplasty surgeons need a sense of beauty to design the target shape, but the shape must comply with the patient's often problematic demands. The only appropriate approach is to suggest two or three shapes that suit the patient's appearance and expectations, and ask the patient to choose between them. The technical difficulty, on the other hand, lies in assessing the anatomical character of the problem and selecting the procedure that will achieve the desired shape. This textbook will help readers to make accurate diagnoses, simulate the target shape employing generally used retouch software, suggest to patients different target shapes, and learn how to choose the best procedure to produce the selected target shape. It will assist in the performance of rhinoplasties which take into account the individual patient's requirements and achieve aesthetic results that guarantee patient satisfaction.
